Abstract
Стаття присвячена розгляду питання щодо фінансування охорони здоров’я України в умовах глобальних викликів. Визначено передумови щодо необхідності реформування національної системи охорони здоров’я, зокрема розглянуто проблемні аспекти її функціонування та фінансування. Окреслено напрямки медичної реформи в Україні. Проаналізовано основні показники фінансування охорони здоров’я України впродовж 2015-2023 рр. та зроблено висновки про їх основні тенденції, зокрема через пандемію COVID-19 у 2020 році та повномасштабне збройне вторгнення рф у 2022 році. Зазначено, що головним пріоритетом реформування системи охорони здоров’я в Україні в умовах війни та світової пандемії COVID-19 має стати модернізація системи фінансування галузі на основі розподілу ресурсів залежно кількості наданих медичних послуг та результатів лікування. Приділено увагу питанню щодо розвитку страхової медицини. Акцентовано на основних її перевагах і також зазначено, що страхова медицина не здатна повністю замінити існуючу модель організації системи охорони здоров’я, адже не може закрити певний набір ризиків для населення та держави. Перспективи розвитку страхової медицини залежать від того, як швидко можна врегулювати визначені ризики та чи вдасться на законодавчому та інституційному рівнях поєднати приватне страхування з існуючою державною системою медичного забезпечення. Представлено заходи, які сприятимуть забезпеченню успішності реалізації реформи охорони здоров’я. Визначено, що основним завданням проведення медичної реформи в Україні в умовах існуючих глобальних викликів має стати підвищення якості медичних послуг, поліпшення здоров’я громадян, забезпечення справедливого розподілу фінансових ресурсів на медицину та зменшення адміністративних витрат.

The article is devoted to consideration of the issue of financing healthcare in Ukraine in the context of global challenges. Prerequisites for the need to reform the national health care system were determined, in particular, problematic aspects of its functioning and financing were considered. The directions of medical reform in Ukraine are outlined. The main indicators of healthcare financing of Ukraine during 2015-2023 were analyzed and conclusions were drawn about their main trends, in particular due to the COVID-19 pandemic in 2020 and a full-scale armed invasion of the Russian Federation in 2022. It is noted that the main priority of reforming the health care system in Ukraine in the conditions of war and the global pandemic of COVID-19 should be the modernization of the industry's financing system based on the distribution of resources depending on the number of medical services provided and the results of treatment. Attention is paid to the issue of the development of insurance medicine. Its main advantages are emphasized and it is also stated that insurance medicine is not able to completely replace the existing model of organization of the health care system, because it cannot cover a certain set of risks for the population and the state. Prospects for the development of insurance medicine depend on how quickly certain risks can be settled and whether it will be possible to combine private insurance with the existing state system of medical care at the legislative and institutional levels. Measures that will contribute to ensuring the success of the implementation of the health care reform are presented. It was determined that the main task of medical reform in Ukraine in the conditions of existing global challenges should be to improve the quality of medical services, improve the health of citizens, ensure the fair distribution of financial resources for medicine and reduce administrative costs.
